The Summit County Emergency Management Agency is conducting a preliminary Public Assistance (PA) Damage Assessment and Individual Assistance (IA) Damage Assessment to survey the storms on August 8, 2024. The damage assessment process may not result in or guarantee supplemental financial assistance for any resident, business, or local jurisdiction.

To submit a Public Assistance Damage Assessment (publicly owned property), please complete the following items:

• PA Damage Assessment Form (attached)
  • Link to FEMA Equipment Rates: FEMA Equipment Rates
• Site Estimate Form (attached)
• Tab B – PA Damage Assessment (attached)
  • Deadline for PA Submissions: The PA deadline will be August 16, 2024.
  • Forward completed PA forms to Joseph St. John at jstjohn@summitoh.net.

To submit an Individual Assistance Damage Assessment (privately owned property) report:

• The IA Damage Assessment can be self-reported via the following link: Resident and Business Self-Report or by downloading the EMA App on your cell phone: EMA App.
  • Deadline for IA Submissions: The IA deadline will be August 14, 2024.

• If you are not the appropriate recipient for this in your jurisdiction, please forward it to the correct person to ensure the assessment is completed on time.
• We will share the collected data with each community and collaborate with them to verify the damage assessments in the field.
• All preliminary PA and IA submissions must be verified before being forwarded to Ohio EMA.
